Melioidosis was studied as a possible biological warfare agent but was never used.\u00a0<em>B. pseudomallei<\/em>\u00a0 is easy to acquire from the environment by inhalation or ingestion of contaminated soil as well as contact through skin wounds. It is highly infectious, antibiotic resistance is readily manipulated, and there is no vaccine available for it. This bacterium is classified as category B priority pathogen by The National Institute of Health and the Centers of Disease Control and Prevention.<\/p>\n<h2>Clinical History<\/h2>\n<p style=\"text-align: justify;\">A 39-year-old male who is diabetic presented with complaints of sudden onset of shortness of breath grade IV. There was a history of high-grade intermittent fever and cough for 10 days. Smooth interlobular septal thickening was noted in the bilateral upper lobes<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-2619 size-full\" src=\"https:\/\/www.kauveryhospital.com\/ima-newsletters\/wp-content\/uploads\/2021\/05\/i3.jpg\" alt=\"\" width=\"390\" height=\"328\" srcset=\"https:\/\/www.kauveryhospital.com\/ima-journal\/wp-content\/uploads\/2021\/05\/i3.jpg 390w, https:\/\/www.kauveryhospital.com\/ima-journal\/wp-content\/uploads\/2021\/05\/i3-300x252.jpg 300w\" sizes=\"auto, (max-width: 390px) 100vw, 390px\" \/><br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-2620 size-full\" src=\"https:\/\/www.kauveryhospital.com\/ima-newsletters\/wp-content\/uploads\/2021\/05\/i4.jpg\" alt=\"\" width=\"417\" height=\"360\" srcset=\"https:\/\/www.kauveryhospital.com\/ima-journal\/wp-content\/uploads\/2021\/05\/i4.jpg 417w, https:\/\/www.kauveryhospital.com\/ima-journal\/wp-content\/uploads\/2021\/05\/i4-300x259.jpg 300w\" sizes=\"auto, (max-width: 417px) 100vw, 417px\" \/><br \/>\nConsolidation with air bronchogram noted in the left lower lobe.<br \/>\nPatchy consolidation also noted in the lingula and right lower lobe posterior segment.<br \/>\nBilateral minimal pleural fluid noted.<br \/>\nNo evidence of pulmonary thromboembolism.<\/p>\n<h2>Based on CT finding, the suggested differentials are :<\/h2>\n<p>Tuberculosis<br \/>\nSeptic embolism<br \/>\nNodular metastasis<\/p>\n<h2>Laboratory Investigations:<\/h2>\n<p>Elevated CRP, D Dimer, procal , altered LFT and RFT<br \/>\nBlood culture showed <strong>Burkholderia pseudomallei<\/strong><\/p>\n<h2>Discussion:<\/h2>\n<p style=\"text-align: justify;\">Melioidosis caused by <em>Burkholderia pseudomallei<\/em> , a gram-negative bacilli, can easily be confused with tuberculosis and various other infections. It can be misdiagnosed clinically, radiologically, and even in the laboratory. The most important factor leading to the diagnosis is a high index of suspicion. Melioidosis should be added in the differential diagnosis when an immunocompromised patient presents with febrile illness, localized suppurative process, and has a travel history or residence in an endemic area. Increased risk of infection is seen in a patient with comorbidities such as diabetes mellitus, excess alcohol intake, renal disease, and chronic lung disease.<\/p>\n<p>The lungs are the most commonly affected organs in melioidosis, commonly presenting as pneumonia.<\/p>\n<p><strong>Abdomen:<\/strong> Affected viscera are often clinically enlarged but not as tender as with other pyogenic infections. The most common manifestations are abscesses. It affects liver , spleen, mesentry, pancreas, prostate., etc.<\/p>\n<p>CNS manifestation\u00a0 includes brain abscesses, meningitis, and encephalitis, particularly involving the brainstem.<\/p>\n<p>Suppurative parotitis and later presents as an abscess.<\/p>\n<p><strong>Pulmonary Melioidosis<\/strong><\/p>\n<h2><strong>Stages<\/strong><\/h2>\n<p><strong><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-2690 size-full\" src=\"https:\/\/www.kauveryhospital.com\/ima-newsletters\/wp-content\/uploads\/2021\/05\/Screenshot_2021-05-15-A-Case-Report-of-Pulmonary-Melioidosis-Kauvery-Hospital-Patient-Newsletters-A-Monthly-Newsletter-fr....png\" alt=\"\" width=\"614\" height=\"662\" srcset=\"https:\/\/www.kauveryhospital.com\/ima-journal\/wp-content\/uploads\/2021\/05\/Screenshot_2021-05-15-A-Case-Report-of-Pulmonary-Melioidosis-Kauvery-Hospital-Patient-Newsletters-A-Monthly-Newsletter-fr....png 614w, https:\/\/www.kauveryhospital.com\/ima-journal\/wp-content\/uploads\/2021\/05\/Screenshot_2021-05-15-A-Case-Report-of-Pulmonary-Melioidosis-Kauvery-Hospital-Patient-Newsletters-A-Monthly-Newsletter-fr...-278x300.png 278w\" sizes=\"auto, (max-width: 614px) 100vw, 614px\" \/><\/strong><\/p>\n<p><strong>Radiologic Manifestation<\/strong><\/p>\n<h2>Acute:<\/h2>\n<p>In <strong>acute septicemic melioidosis<\/strong>, the most common radiographic appearance is disseminated nodular lesions representing hematogenous spread. Rapid disease progression can result in abscess formation with cavitation.<\/p>\n<p>In <strong>acute nonsepticemic pneumonic melioidosis,<\/strong> the most common radiographic pattern is similar, with alveolar infiltration and consolidation with or without cavitation<br \/>\nPleural effusion or empyema is occasionally seen\u00a0in acute stage.<\/p>\n<h2>Subacute:<\/h2>\n<p>Nonspecific patterns of pulmonary infiltration, including nodular, alveolar, or mixed infiltration\/consolidation, with or without cavities<\/p>\n<h2>Chronic:<\/h2>\n<p>Mixed infiltration with cavities and fibronodular lesions associated with linear densities can be observed, which strongly mimics reactivation pulmonary tuberculosis.<\/p>\n<h2>Subclinical Form:<\/h2>\n<p>This type of presentation may be silent for many years, and infectious reactivation can present with one of the clinical forms when host resistance is compromised.<\/p>\n<h2>Treatment<\/h2>\n<p>Empiric anti-microbial coverage is generally recommended if there is suspicion of melioidosis, especially in the septicaemic form.<\/p>\n<h2>Conclusion<\/h2>\n<p style=\"text-align: justify;\">Melioidosis is becoming an increasingly global infection. Melioidosis can present with a wide spectrum of manifestations and can affect any part of the body.
