Superficial infections of the most distal part of the pulp skin are known as apical infections. Apical infections are different from felon in that the palmar pad is not involved. The term Felon should be reserved for those infections involving multiple septal compartments and causing compartment syndrome of the distal phalangeal pulp. The most common organism grown from felons is S. aureus. Gram negative infections have also been reported. Though this condition is uncommon, it is typically seen in immunocompromised.<\/p>\n<h2><strong>Case Reports<\/strong><\/h2>\n<ul class=\"list-icon check\">\n<li>A 51-year-old male with uncontrolled diabetes presented with a history of pain and swelling left index finger for the past two weeks. The patient noticed discoloration and worsening pain for the past two days. On examination, he had pulp space infection with TP and MP region gangrene with possible spread along the flexor tendon sheath (Fig: 1 &amp; 2). Emergency debridement with amputation of the gangrenous TP region was done under antibiotic cover, and after dressings for two days, cross finger flap (CFF) was done (Fig: 3) to cover the volar aspect of the middle phalanx (Fig: 4). Flap division and inset was done after a gap of two weeks. The patient was given physiotherapy, and at the end of treatment, he could achieve a good range of movement at the PIP joint and a functioning finger (Fig: 5 &amp; 6).<\/li>\n<li>A 74-year-old male presented with history of pain, swelling, redness, and pus discharge in the tip of left index finger past one week. He was a known CKD patient, on regular dialysis, thrice a week. He was not a diabetic. On examination, the volar skin was necrotic with preserved movements at PIP and DIP joints (Fig: 7). Debridement was done, and the necrotic tissues were excised preserving the insertion of FDP tendon (Fig: 8). Cross finger flap was done (Fig: 9), and patient achieved full ROM at DIP joint and a good pulp tissue at the end of treatment.<\/li>\n<li>A 62-year-old male diabetic with history of injury to the right mid finger, following which he developed pain, swelling, and redness followed by blackish discolouration of the tip. On examination he had gangrene of the TP region, for which debridement with amputation of finger at DIP joint level was done. FDS tendon was preserved and cross finger flap was done to cover the exposed tendon (Fig: 10 &amp; 11). Patient recovered well with a functioning ray and a good range of movement at PIP joint (Fig: 12).<\/li>\n<\/ul>\n<h2><strong>Case Discussion<\/strong><\/h2>\n<p>Felons account for 15% to 20% of all hand infections. A felon is characterised by severe throbbing pain, tension and swelling of the entire distal phalangeal pulp. With the progression of swelling and tension there is compromised venous return leading to microvascular injury and tissue necrosis and abscess formation. History of penetrating injury may be there at times. The expanding abscess breaks down the septa and can extend to the phalanx or the skin. If decompression does not occur it is possible that the vessels are obliterated and a slough of the pulp will result. Other complications of an untreated felon include osteomyelitis of distal phalanx, pyogenic arthritis of DIP joint and flexor tenosynovitis and proximal extension.<\/p>\n<h2><strong>Treatment<\/strong><\/h2>\n<p>Treatment of felon should be directed toward preserving the function of distal phalanx which includes fine tactile sensibility and stable durable pinch.<\/p>\n<p>In the early cellulitic stage, it is treated with hand elevation and appropriate antibiotics. Immobilization may make the patient more comfortable. Surgical drainage is done by making a unilateral longitudinal incision when there is fluctuance in the pulp. Incisions are made in such a way as to avoid injury to vessels and nerves. Also, the flexor tendon sheath should be preserved. Although it is preferred to avoid placing the incision on the pinching surface, the incision should always be placed on the side of maximal tenderness. When possible, it is made on the ulnar side of the second to fourth digits and on the radial side of the thumb and little finger. The incision is started dorsal to and 0.5cm distal to DIP joint flexion crease. The cavity is enlarged until the adequate evacuation is achieved.<\/p>\n<h2><strong>Role of a Plastic Surgeon in Hand Infections<\/strong><\/h2>\n<p>Timely referral of hand infections to a plastic surgeon before compartment syndrome sets in is crucial. Even in the stage of spreading infection, early intervention in the form of debridement arrests further proximal spread. By timely intervention in the form of surgery, we can prevent pyogenic flexor tenosynovitis and septic arthritis of finger joints.
