High Cut-Off Dialyzers: Expanding frontiers in extracorporeal blood purification

High cut-off (HCO) dialyzers are specialized extracorporeal membranes designed with larger pore sizes and molecular weight cut-offs of approximately 45–60 kDa, permitting enhanced clearance of middle and large molecular weight solutes. Unlike conventional high-flux membranes, HCO dialyzers effectively remove pathogenic proteins such as serum free light chains (sFLCs), myoglobin, β2-microglobulin, and inflammatory mediators while preserving most essential plasma proteins. Their primary clinical application has been in multiple myeloma-associated cast nephropathy; however, recent evidence supports an expanding role in severe rhabdomyolysis-associated acute kidney injury (AKI).

The Silent Bleed: Delayed Hypovolemic Shock from Occult Lower Limb Hemorrhage Following Blunt Trauma

Abstract

Trauma-related hemorrhage is a leading cause of preventable mortality. While torso bleeding is commonly prioritized, extremity hemorrhage can be equally life-threatening and often under-recognized.

we present a 59-year-old male presented following a road traffic accident with bilateral lower limb injuries. Initial assessment revealed stable vitals with no active external bleeding. Despite negative EFAST and imaging for thoracoabdominal injuries, the patient developed sudden hypotension. Serial reassessment revealed occult hemorrhage from a left lower limb vascular injury involving the posterior tibial artery.

A Mystery Solved: Unmasking Gallbladder Sludge as a Cause of Idiopathic Recurrent Pancreatitis

Abstract

Recurrent acute pancreatitis with no identifiable etiology poses a significant diagnostic and therapeutic challenge. Gallbladder sludge is an under-recognized cause of so-called idiopathic pancreatitis and may be missed on routine imaging. We report the case of a 32-year-old male with recurrent acute pancreatitis in whom extensive evaluation, including metabolic, autoimmune, and genetic workup, was inconclusive until repeat fasting ultrasonography revealed gallbladder sludge. The patient subsequently underwent laparoscopic cholecystectomy, following which he remained asymptomatic with no recurrence on follow-up. This case highlights the importance of a systematic etiological evaluation in recurrent pancreatitis and underscores the role of repeat imaging in identifying occult biliary pathology.

Ambulatory Blood Pressure Monitoring (ABPM): Bringing Precision to Hypertension Management

Hypertension continues to be one of the leading modifiable risk factors for cardiovascular disease, stroke, chronic kidney disease, and premature mortality in India. Despite advances in treatment, many patients remain either undiagnosed or inadequately controlled. One of the major reasons is the reliance on isolated clinic blood pressure measurements, which may not accurately reflect the patient’s true blood pressure profile. Ambulatory Blood Pressure Monitoring (ABPM) has therefore emerged as an invaluable tool in modern hypertension practice.

Anaesthetic Management of Epidermoid Tumor Excision in a Patient with Split Cord Malformation Using Remifentanil–Rocuronium Based General Anaesthesia: A Case Report

Abstract

Background:

Intracranial epidermoid tumours are rare congenital lesions that grow slowly and often present with neurological symptoms due to compression of adjacent neural structures. The presence of congenital spinal anomalies such as split cord malformation adds complexity to anaesthetic management because of concerns regarding neurological injury, positioning, and maintenance of spinal cord perfusion.
