Tumors at Every Corner: Unraveling an NF2 Mystery
Case History:
A 26-year-old male presented with a 3-year history of dysphagia, along with intermittent neck stiffness, giddiness, and neck pain persisting over the past 2 years.
Initial investigations included MRI brain and spine, which revealed:

THE ANOMALOUS COMPLICATION OF DENOSUMAB
Introduction:
Denosumab is used to treat osteoporosis in postmenopausal women, to increase bone mass in men with osteoporosis, and to treat bone loss in people receiving certain cancer treatments. It works by binding to a protein called RANKL, which is involved in the development and activity of osteoclasts (cells that break down bone tissue), thus reducing bone loss. Denosumab has been approved by the US Food and Drug Administration (FDA) for the treatment of bone loss and skeletal-related events in patients with advanced carcinoma of the breast. This is because breast cancer cells can spread to the bone and cause bone destruction, leading to pain and increased risk of fractures.

OCTREOTIDE-LAR IN OVERT OBSCURE GI BLEED (OOGIB)
Introduction:
Overt Obscure gastrointestinal bleeding (OOGIB) is defined as bleeding from the gastrointestinal tract that persists or recurs after a initial evaluation using bidirectional endoscopy and radiologic imaging with small-bowel radiograph. Diagnosis of OOGIB may include angiography (a procedure that involves injecting contrast dye into blood vessels to identify the source of bleeding), capsule endoscopy (a procedure that involves swallowing a camera capsule that takes pictures of the digestive tract) or double balloon endoscopy. Treatment options depends on the location and severity of the bleeding and may include medications, endoscopic procedures and surgeries.

Chorea in a Renal Transplant Recipient: A Rare Manifestation of Metabolic and Vascular Insults to the Basal Ganglia
Abstract:
We report a rare case of left-sided choreoathetosis in a 60-year-old renal transplant recipient associated with a recent hyperglycemic episode and multiple chronic lacunar infarcts involving the basal ganglia. This case highlights a complex interplay between metabolic dysregulation, vascular compromise and immunosuppression, presenting as a hyperglycemia-related movement disorder in a post-transplant setting.

A Vision at Risk: Managing a Cavernous Carotid Aneurysm with Flow Diversion
Introduction
Giant intracranial aneurysms, particularly those involving the cavernous segment of the internal carotid artery (ICA), present significant diagnostic and therapeutic challenges due to their size, location, and potential for compressive symptoms. These aneurysms can cause progressive neurological deficits such as visual disturbances, cranial nerve palsies, and headaches due to their mass effect on adjacent neurovascular structures. Conventional surgical options carry high morbidity, especially in deep-seated or complex lesions. Recent advances in endovascular techniques, particularly the use of flow diverter stents, have transformed the management of such lesions by offering a minimally invasive approach that promotes aneurysm thrombosis and vessel wall reconstruction while preserving the parent artery. This case highlights the effectiveness of flow diversion in treating a complex cavernous ICA aneurysm with excellent clinical and radiological outcomes.

Management of patient with Ralstonia Mannitolytica infection in our ICU- A case report
Abstract:
Ralstonia mannitolilytica is a non-fermentative, Gram-negative bacillus increasingly recognized as an opportunistic pathogen in healthcare settings, particularly among immunocompromised individuals. Its resilience in aqueous environments and resistance to standard sterilization methods make it a formidable nosocomial pathogen. This article explains our management of Ralstonia mannitolytica infection in our ICU.
