A middle-aged male driver from Brunei Darussalam presented with a complex multi-system medical history involving a left cerebral glioma causing right-sided hemiparesis, a congenital cardiac defect (ASD), and bilateral Avascular Necrosis (AVN) of the femoral head.

The patient first noticed a sudden onset of giddiness, right-sided facial weakness, dysphasia (difficulty speaking), and weakness in his right upper and lower limbs.
