Dr AravindhanWelcome to the July 2015 edition of Kauvery Newsletter! My best wishes to every doctor this Doctor's day (01 July).

I am extremely happy seeing the positive response to our first newsletter which came out last month.

June was full of exciting activities in Kauvery hospital. One of the most important one was the launch of our 'Orthogeriatrics' department - the first of its kind in India. We also conducted medical camps and had public talks covering medical issues in many forums.

This month's newsletter also includes a video talk by our nephrologist on how diabetics can protect their kidneys. We hope to cover many such interesting topics in future.

July is also full of activities in our hospital. Check our upcoming events section for details.

Diabetes - A Major Risk Factor for Kidney Disease



Diabetes is the commonest reason for chronic kidney failure. Longstanding uncontrolled diabetes harms blood vessels within the kidney. Microalbuminuria (leakage of small quantities in urine) is the earliest sign of kidney damage. Protein leakage increases gradually following which hypertension and renal failure may develop. Dialysis and kidney transplantation can be safely performed in diabetics with kidney failure.

Q.
Doctor, I have a small lump on the left side of my neck and I am worried it may grow very big. I do not have any other symptoms like pain, fever, itching, fatigue, sweating at night, or weight loss. Should I be concerned?

A.
On physically examining the lump, I can see that you have a walnut-sized lump on the left side of your neck which could possibly be a lymph node. The function of the lymph nodes is to produce and store cells, which help fight infections and disease. The human body has around 600-700 lymph nodes. It is part of the lymph system, which is the defense mechanism against illness.

A lymph node swelling is a symptom of many types of illnesses like, cold or fever, allergic reaction to medications, ear infections, gingivitis, infected tooth, tuberculosis, toxoplasmosis, tonsillitis, syphilis, strep throat, rheumatoid arthritis, measles, non-Hodgkin’s lymphoma, Hodgkin’s lymphoma, mouth sores, metastasized cancer, HIV, lupus or leukemia.

By clinical examination, the lump feels hard and you do need to have a biopsy done to rule out malignancy. Even otherwise, you may also like to have the lump removed for cosmetic reasons.

The Surgeon will perform an open biopsy to remove the lump. You will be administered a local anesthesia. A very small incision will be made on the side of your neck to remove the offending lump. The procedure will take about 30 to 45 minutes. It is an outpatient procedure. Post-surgery, the lump will be sent to the pathology lab to be examined under a microscope. A report will be given to you. Based on the report, treatment options can then be considered.



Q.
Doctor, I often find it difficult to bend or stand for more than an hour, because of pain on the right side of my hip. I sometimes limp. Is there something seriously wrong?

A.
Pain in the hip sometimes is a cloak for lower back ache and most hip and back problems are due to ordinary wear and tear on the body. Some of the common causes of hip pain are:

1. Arthritis: This is the most common cause, particularly osteoarthritis. As you age, the wear and tear on your body and the ball and socket joint in the pelvic region starts to wear out. The arthritic pain in the hip is often felt in the groin or in the front of the upper thigh region. This is generally due to inflammation in the joint or because of stiffness.

2. Hip Fractures: This is a common feature in many older people, especially due to osteoporosis.

3. Tendinitis or bursitis: The muscles in the hip are connected to the joint, by many tendons. Inflammation in these tendons can be easily caused either due to overuse or due to vigorous activities. One of the most common cause of tendinitis, especially in runners, is iliotibial band syndrome. Bursitis is another common cause of hip pain. Bursae are sacs filled with fluid, and like the tendons cushion the bony part of the hip. When the bursae become inflamed due to overuse, it causes pain, whenever the hip joint is moved.

Pain in the hip or lower back, especially in women could be for gynecological and back issues. Depending on your age and other health issues, the pain in your hip or lower back could be from other systems in your body. It is therefore, important to have yourself checked out by an Orthopedician and treated.

Pregnant Patients - Be Aware of Preeclampsia
In many developing and third world countries, the awareness and consequences of preeclampsia is little known, even though its effects are far reaching; as far as towns, villages and hamlets where state-of-the art- medical technology is yet to anchor itself.

Preeclampsia or toxemia as it was previously known is a condition that develops in pregnant women. It normally develops in first time pregnancies and occurs in the second or closer to the end of the second trimester. If left untreated preeclampsia can lead to a serious condition called eclampsia, which would expose both the mother and the unborn child to health risks and in rare cases, even cause death. There is no cure for preeclampsia but when diagnosed and treated in the early stages, it can be managed.

Symptoms of Preeclampsia include:
• Swelling of the feet, hands and face (some swelling of the feet and ankles, is considered normal)
• High blood pressure
• Protein in the urine
• Severe headaches
• Rapid gain in weight caused by increased bodily fluids
• Pain in the abdomen
• Slow reflexes
• Dizziness, excessive nausea and vomiting
• Reduced output of urine
• Changes in vision - flashing lights and floaters in the eye(s), blurry vision

In some cases, the symptoms may not be visible yet you could have preeclampsia. Hence, monthly check-ups are very important. If you experience any of the symptoms listed above then you must seek medical help immediately.

Consequences of Preeclampsia
• It can prevent blood supply to the placenta, which could affect the baby’s body size, causing it to be born very small

• It is also one of the leading causes for premature births

• Other complications include learning disabilities in the child, epilepsy, hearing defects, vision problems and cerebral palsy

• Complications, a mother-to-be (though rare) may face are issues like seizures, heart failure, stroke, water in the lungs, bleeding from the liver, reversible blindness and bleeding after the birth of the baby

• It can also cause still birth. This happens when the placenta gets separated from the uterus - also known as placenta abruption

Treatment Options
There is only one treatment option for mothers-to-be who are affected by preeclampsia / eclampsia and that is for the doctor to deliver the baby, after taking into consideration the following factors:
• How well the baby is doing in the womb
• The severity of the affliction
• How far along the pregnancy is

During your pregnancy, stay tuned into your instincts and trust them. Call your doctor or healthcare provider, and clear your doubts. Help counter preeclampsia with early detection, and help prevent the worst outcomes. Share this important awareness information, spread the word, be the harbinger.

Kauvery Hospital Launches the Country’s First-of-its-Kind Exclusive Orthogeriatrics Department
Friday, June 26, 2015

Kauvery Hospital has opened Department of Orthogeriatrics for comprehensive orthopedic care for senior citizens here on Friday. Eminent geriatric specialist Dr V S Natarajan inaugurated the department.

The department combines medical specialities of orthopedic and geriatrics to provide holistic care with the top-of-the-line infrastructure and patient-friendly amenities.

Stressing on the importance of comprehensive geriatric care, Dr Aravindan Selvaraj of the Kauvery Hospital said, 'United Nations Population Fund (UNFPA) in its study brought out jointly with Help Age International points to increase of people above 60 years of age to 323 million by 2050 from its present 100 million, which constitutes 20 per cent of India's total population by then and it further predicts the number of over 80s to increase seven fold'.

'It is also of concern that the study discloses that around one-fifth of the aged people live alone or only with spouses in both rural and urban areas. All this shows that there is a need to strengthen geriatric care so that healthcare demands of growing senior citizens can be met,' he added.

The Orthogeriatrics Department is spread over 12,000sq ft and houses 15 exclusive in-patient beds, elders friendly amenities like low level cots and bathrooms with grab bars, spacious corridor for physiotherapy and rehabilitation apart from advanced medical equipment.

Cardiology Camp at Mandaveli

A free cardiology screening camp was conducted at MTC Mandeveli on the 12th June 2015. Our medical officer, Dr. Naveen evaluated people for cardiac illness. More than 90 people attended the camp and got benefited.
   
CME Programme at Pondicherry

Dr Aravindan Selvaraj spoke on surgical management of frozen shoulder and Dr Iyappan Ponnuswamy discussed minimally invasive interventional options in modern clinical practice at a CME programme conducted by the Indian Medical Association, on 14th June 2015, at Le Royal Park Hotel, Pondicherry. The speakers were honoured by Directorate of Medical Services Mr.Raman and Ex-Directorate of Medical Services Mr.Dilipkumar Paliga.
   
Eldams Road Camp

A multispecialty camp including orthopaedics, cardiology, gynaecology and diabetology was conducted at Eldams road on 21st June 2015. International Yoga day was celebrated before the camp and the patients were advised to do yoga.

254 patients got benefited through the camp.
   
Talk on Renal Diseases among Industrial Workers

Dr.R.Balasubramaniyam, our Senior Consultant Nephrologist, gave a lecture to more than 100 participants on renal diseases among industrial workers at Olive Hall, Savera hotel on 21st June 2015.
   
Interactive programme on Epilepsy by ABHAYA
(Support group for people with Epilepsy)


An interactive programme on epilepsy was conducted at 7th Floor conference hall of our hospital on 23rd June by ABHAYA - a support group for people with epilepsy. The event was headed by leading neurologist and neurosurgeon, Dr. Prithika Chary.

Doctors of Kauvery Hospital performed a Medical Clowning program for the patients to understand epilepsy.

Student Experience

Every year, the students from US and UK, who aspire to become doctors spend a month in Kauvery hospital as clinical observers for work experience. Many of them get so much motivated observing our doctors and at the end of one month, their perspective about the medical profession completely changes. One of the students from London, who was not sure whether to do medicine or not, got so inspired after watching cardiac surgery and now he wants to become a cardiologist. It gives enormous satisfaction for we doctors as well to see these young men and women become passionate professionals. Upon completion of my first year in college, I was keen to pursue a summer study that further provided me exposure to the daily aspects of the medical profession. As I choose to further narrow down my areas of focus from a study perspective, it was important for me to get an early exposure into the different shades of medical profession. I was lucky to be offered the opportunity at Kauvery to be a part of 2015’s summer study under the guidance of some of the best minds in the profession. My experience at Kauvery was a comprehensive one. Be it a field of expertise, a type of ailment, or a set of internal processes, I was fortunate enough to be able to interact with multiple departments and experts within the hospital. Under the guided supervision of the experts, I gained valuable insight and experience that I will cherish beyond my professional needs. Though I am unsure about the specific type of medical professional I will be, I will always remember this as the environment in which I was first able to sink my teeth in. Hopefully, I return here in future to contribute to the professional environment here at Kauvery Hospital. I learned so much from all the physicians and surgeons about what it truly means to be a medical professional. To see, to understand, and to help. I will keep this and all that I have learned in mind as I move forward with my future aspirations. Thank you for a wonderful summer of learning.

Recipe Corner

Egg Roll - A power packed snack

If you want to power pack yourself with proteins, vitamins, minerals, good fats and lots of other nutrients, then you must eat an egg a day. Tired of fixing yourself an omelette or hard boiling an egg? Here is a recipe which can keep you going for many hours. The Egg Roll is a complete meal and is truly loaded with lots of goodness. An Egg-cellent recipe indeed. Why don’t you try this today!

Prep time - 30 minutes; Serves - 2

Ingredients:
• Whole Wheat Flour (or Maida or Corn Flour) - 2cups
• Eggs-5
• Spring Onions – 2 tablespoons, chopped finely
• Green Chilies – 2 small (depending on spice level required)
• Red Capsicum – 1 medium sized, minced
• Corn Kernels – 1/3 cup
• Chicken (cooked and shredded) – 2 cups minced, cooked (optional)
• Spinach leaves – 2 tablespoons, chopped finely
• Cumin Powder (Jeera) – ½ teaspoon
• Red Chili powder – ¼ teaspoon
• Fresh Green Coriander or Parsley – 1 cup, chopped finely
• Refined or Vegetable Oil - 2 Table spoons
• Turmeric Powder (or freshly grated turmeric) – ¼ teaspoon
• Salt to taste
• Water for mixing the dough

Preparation:
1. Put the flour and salt (to taste) in a bowl. Mix them and then add water to make the dough. Knead well and make 4 balls out of the dough. Roll out the balls into medium sized parathas.

2. Beat the eggs in a separate bowl with a pinch of salt and turmeric powder.

3. In a Kadai, heat 2 tablespoons of oil, sauté the spring onions, green chilies, spinach, red capsicum, and corn. If you want to use chicken, add it to the mix. Add salt to taste, turmeric powder, red chili powder, cumin powder and cook until done. If you want to use chicken, add it to the mix.

4. On a tawa, ladle out the beaten egg to make an omelet. Place one of the rolled out paratha on top of the omelet, add a small teaspoon of oil and cook on low fire. Turn over to cook the paratha. Remove and place on a plate, egg side up once done.

5. Spread a spoon of filling, mix of red capsicum, cooked chicken, spring onions, etc, on top of the egg and spread evenly.

6. Garnish with chopped Parsley or Coriander leaves.

7. Fold the paratha at both edges inwards (1/4 inch) to hold the filling and then roll the paratha to make a roll.

Serve with Hot and Sweet Tomato Sauce. (The filling could be made with variations of vegetable and meat)
