What Is Pulsed Field Ablation, and Why Is It a Game Changer?

Heart rhythm disorders, particularly atrial fibrillation (AF), affect millions of people globally and are among the most common causes of palpitations, stroke, and heart failure. For years, the standard treatments, radiofrequency ablation (heat-based) and cryoablation (cold-based), while effective, carried inherent risks to surrounding structures such as the oesophagus, phrenic nerve, and pulmonary veins.

Pulsed field ablation (PFA) rewrites that story.

Using precisely controlled electrical pulses, a process known as electroporation, PFA selectively targets and eliminates only the abnormal heart tissue responsible for irregular rhythms. It does so without heat, without extreme cold, and with remarkable precision. This is the first time in Tamil Nadu that both platforms have been introduced simultaneously, and the first time a 3D mapping-integrated PFA system has been made available in the state. To mark the occasion, leading international electrophysiology experts from the United States travelled to Chennai to collaborate with the Kauvery team and support the first procedures, a powerful statement of the global-standard care now available locally.

Alwarpet Centre, 14th June 2026: Medtronic PulseSelect™

A 70-year-old woman, a known case of diabetes mellitus and hypertension, had been living with troubling and recurring episodes of palpitations. A Holter monitor study confirmed that she was experiencing intermittent episodes of atrial fibrillation, leading to a diagnosis of paroxysmal atrial fibrillation.

Given her age and multiple comorbidities, treatment needed to be both effective and safe. After a thorough evaluation, the Kauvery Heart Rhythm team at Kauvery Hospitals, Alwarpet, recommended a cardiac EP study and pulsed field ablation for pulmonary vein isolation, performed under intracardiac echocardiography (ICE) guidance, a technique that provides real-time imaging of the heart from within, allowing for precise catheter navigation and tissue targeting.

The procedure was performed using the Medtronic PulseSelect™ system and was a resounding success. The entire procedure was completed in just 45 minutes, significantly faster than conventional radiofrequency ablation or cryoablation, which can often exceed two to three hours. The patient was monitored overnight and discharged the very next day, recovering comfortably and free from the palpitations that had plagued her for so long.

Vadapalani Centre, 19th June 2026: Varipulse™ by Johnson & Johnson

A 63-year-old man with a known history of hypertension and coronary artery disease (CAD) had been experiencing recurrent episodes of non-sustained palpitations. His Holter study confirmed episodes of atrial fibrillation, and he was diagnosed with paroxysmal atrial fibrillation.

Given the complexity of his cardiac history, particularly his CAD background, a carefully planned approach was essential. The team at Kauvery Hospitals, Vadapalani, recommended a cardiac EP study and PFA-guided pulmonary vein isolation, with intracardiac echocardiography guidance to ensure precision throughout.

The procedure was performed using the Biosense Webster Varipulse™ platform, integrated with the centre’s advanced 3D cardiac mapping system allowing the electrophysiology team to create a detailed, real-time map of the heart’s electrical activity and guide the ablation with exceptional accuracy. The procedure was completed successfully, and the patient was discharged the following day with no complications and significant relief from symptoms.
