Abstract

Continuous professional development in patient safety is essential for minimizing preventable harm and promoting high-reliability healthcare systems. A structured Patient Safety Workshop was conducted at Kauvery Hospital, Electronic city on 3rd February 2026 with the objective of reinforcing safety culture, enhancing risk management practices, and strengthening interdisciplinary communication. The program integrated didactic lectures, interactive discussions, and case-based learning. The workshop demonstrated measurable improvements in participant awareness and reinforced institutional commitment to transparent reporting and continuous quality improvement. The workshop brought together a multidisciplinary team of healthcare professionals united by a shared Mission putting Patient Safety first.

The workshop was conducted on 3rd February 2026 from 08:00 AM to 02:00 PM at the 5th Floor Training Hall, Kauvery Hospital. Participants included consultants, nursing professionals, quality officers, and allied healthcare staff. The method used in this workshop are pre-assessment evaluation, Didactic lectures, Case-based discussions, Interactive scenario analysis, post-session knowledge reinforcement quiz & Scientific Sessions.

Foundations of Patient Safety

The first session, led by Ms. Vijayakumari (Nurse Educator) and Ms. Pavithra (DNS). The introductory session outlined fundamental safety principles, including system-based error analysis, human factors engineering, and International Patient Safety Goals (IPSG). Their presentation highlighted the shift from individual blame to system-based improvement and a critical mindset in modern healthcare.

Communication and Teamwork: The cornerstone of safety

Effective communication & teamwork plays a vital role in preventing errors & strengthen the team. Ms. Bhuvana & Ms. Vijayakumari engaging session on the ISBAR framework & Enhanced Communication in healthcare. A structured session on the ISBAR framework (Identify Situation, Background, Assessment, Recommendation) & Enhanced Communication (AIDET, LEAP & 7 Cs of Communication) highlighted the role of standardized communication, enhances clarity, reduces misunderstandings, and improves team coordination. Through interactive discussions and practical examples, participants are explored how teamwork directly impacts on patient outcomes.

Strengthening clinical safety

Ms. Shalini HS (CNO) addressed medication and clinical safety, focusing on Prevention of high-alert medication errors, Safe prescribing and administration practices, Documentation accuracy, Monitoring and adverse drug event prevention and Clinical case scenarios facilitated contextual learning. Her session reinforced the importance of vigilance, documentation accuracy, and adherence to standardized protocols.

Proactive risk management

Dr. Naganand (Medical Manager) engaged the session on risk assessment and mitigation strategies encouraged the participants to move beyond reactive responses. Emphasizing proactive hazard identification, he discussed the importance of safety audits and structured risk management frameworks in creating resilient healthcare systems.

Learning from incidents

A key highlight of the workshop was the session on incident reporting and Root Cause Analysis (RCA) by Ms. Anitha (ANS). She emphasized fostering a non-punitive reporting culture where near misses and adverse events are viewed as learning opportunities. The concept of a “just culture” was highlighted to promote transparency and accountability.
