Robotic redo extended hellers cardiomyotomy: An ultra-short report with procedure video

Case Presentation

A 35 year aged male with known case of Achalasia Cardia. Underwent endoscopic Botox twice, dilatation 3 times and POEM (Peroral Endoscopic Myotomy) once.

Complaints

Presented with dysphagia and reflux symptoms and recurrent achalasia cardia after evaluation

Management

Underwent Redo Robotic Extended Hellers Cardiomyotomy.

Discharged 2nd Post op day on liquids and 7th POD on Soft diet.

Discussion

After extensive literature search, this is the first case report of Redo Robotic Extended Hellers Cardiomyotomy performed for recurrent achalasia cardia after failed multiple endoscopic procedures.

POEM if performed properly is considered as procedure of choice for Type 1 Achalasia recently as it’s done by endoscopy.

The chances of reflux after POEM is more compared to Surgery, but it is usually managed by PPIs.

After performing such cases by Laparoscopic approach earlier, the Robotic platform was an added tool that helped in better vision and delenition of anatomy in area with fibrosis.
