Successful Mechanical Thrombectomy: Two case reports

Case Presentation

A 50-year-old male, with acute onset right dense hemiplegia and aphasia

CT brain and angio showed no bleed and critical left ICA stenosis, with a floating thrombus.

The time of onset was 2 pm and he arrived to ER at 4 pm. The patient was thrombolysed and immediately taken up for Mechanical Thrombectomy at 5 30 pm.

Management

Aspiration thrombectomy was done which cleared the thrombus but still ICA stenosis was there

So decided to stent it.

Post stenting, near total resolution of stenosis

The Puncture to Recanalization and stenting was done within 50 min

Case 2

A 54-year-old male with left dense hemiplegia came in window period

IV thrombolysed, and CT angio showed Rt ICA occlusion.

Since catheter lab was occupied in Alwarpet, shifted him to Vadapalani (12 min in ambulance). Took him up for mechanical thrombectomy

Management

Right ICA angio showed occlusion of distal ICA so Solumbra technique was used for thrombectomy.

TICI 3 recanalization done in 3 phases
