Case Presentation

A patient of Paradoxical Embolism (causing recurrent stroke), misdiagnosed abroad as Cryptogenic Stroke was found to have Patent Foramen Ovale (PFO), with right to left shunt, in our center.

It was easily closed with Amplatzer PFO Occluder, and further crippling stroke episodes were prevented.

This is an indication for closing a PFO which can be very rare cause of recurrent stroke.

Conclusion

Cryptogenic stroke is an ischemic stroke with no identifiable cause after a thorough diagnostic evaluation, which typically includes brain imaging (CT/MRI), vascular imaging (CTA/MRA/carotid Doppler), cardiac monitoring (ECG, Holter, echocardiogram) and blood tests to rule out prothrombotic states or other causes.

PFO is a common suspected contributor, especially in younger patients.

PFO closure is recommended in patients with a history of cryptogenic stroke when no other cause of stroke is found after comprehensive evaluation.
